Here are a list of some of the forms you may need to submit for various administrative tasks.

• 2007 KSA Career Fair Guide
  A 9-page guide to the firms who will be attending the 2007 KSA Career Fair on February 22nd.

• City and Regional Planning Minor
  The minor in City and Regional Planning consists of a minimum of 24 credit hours. This worksheet helps you keep track of those classes as you work toward the minor.
• Course Enrollment Premission
  Fill out the form completely - including the call number and other course information - and obtain the appropriate signatures before bringing this form to the advising office.
• Fresh Start Form
  
• Honors Program Application
  Eligibility for Honors status requires that a student maintain a minimum cumulative GPA of 3.4 or higher for two consecutive quarters. This application should be submitted to your academic advisor for approval.
• KSA Graduation Application

• Post 7th Friday Drop / Withdraw
  As stated on the form, "a lack of preparation or dissatisfaction with the course are not acceptable reasons to drop a course or withdraw this late in the period." Be prepared to submit a doctor's note or other appropriate documentation. Petitions for post-7th Friday drops or withdraws are rarely approved.
• Reinstatement

  Submit this form in person to the University Registrar's office (3rd floor of Lincoln Tower)
• Request for Independent Study
  Submit this form to your academic advisor after obtaining permission from the instructor who will be supervising your independent study.
• Transfer Evaluation
  If you plan to take a course at another college, it is important that you find out how the credit transfers back to Ohio State and applies toward your degree requirements. This form will help you through that process.
• Undergraduate Petition for Course Credit
  Please attach this form to your petition, course syllabus, and advising report. Submit all materials to the KSA Advising Office, 100 Knowlton Hall.
